Thank you for your reply.For a disabled person permanent placard (if you had a temporary one), you must complete the Form REG 195 and get the doctor to complete their part. See: http://www.dmv.ca.gov/portal/wcm/connect/aebb95cd-c20a-49bd-bc13-dd74120044fc/reg195.pdf?MOD=AJPERES Once completed, you can mail the form into DMV If you had a permanent disability placard, then there is no need to renew, it renews automatically. If you have not received the renewal, then you need to contact DMV at 1-***-***-**** or to get a replacement of a lost or damaged placard, you can fill out a Form 156 and mail it to DMV Placard, PO Box 942869 Sacramento,CA###-##-#### (see: https://www.dmv.ca.gov/portal/wcm/connect/78848945-2d42-4903-a38b-329805062c7e/reg156.pdf?MOD=AJPERES). Those are the two ways to get the new placard.
