Humana has canceld my policy for "failure to pay", which

Humana has canceld my policy for "failure to pay", which I have never missed a payment or been late. I gave them supporting document from canceled checks from my bank way back August, and they still canceled my policy. My automatic payments are sent from my Wells Fargo bank account. The first one was sent in error to the wrong Humana address, however the check was cashed by Humana. I have met my deductible and paid almost $500/month for my coverage since July.  My policy was set to end at the end of december and they are not cooperating to reinstate my insurance.  I have big medical appointments set that will now not be covered.

What can I do ASAP?
