Customer Question

I am filming a documentary and a lot of the subjects are asking for a copy of the interview for their own records...
I dont mind them using it on their own website - but what is the best way to handle this
I would like them to credit it wherever they display it
That they cannot give it to anyone else to use
Is there a standard form / agreement that I can use or else how much would you charge to create one?
